  • All memberships have a $99 enrollment fee

  • Memberships paid yearly or monthly (Not billed to Insurance)

  • 3 month minimum commitment on all accepted memberships 

  • Initial appointments cannot be scheduled on the same day as sign-up. 

  • Once you’ve completed your enrollment form, it will be reviewed and if applicable, confirmed. Your enrollment fee is not charged until we confirm your membership. We will contact you to via email arrange your first appointment.

Conditions not treated

  • Initial evaluation and diagnosis for ADD/ADHD– We can help refer you to a neuropsychologist for appropriate testing. If you have had testing done and documentation confirming diagnosis, we are happy to help manage your ADHD medications.

  • Pain or Complex Psychiatric Management – We do not prescribe controlled substances (e.g. narcotics, benzodiazepines) for long term management. We are happy to help coordinate your referral to a pain management or psychiatric specialist for those prescription needs.

  • Fracture Care – We can assess and order X-rays and diagnostics then refer to an orthopedic specialist
